Your Specific Responsibilities:

This is professional work planning, implementing and conducting health promotion and public health education programs for clinics, schools and community groups. This position is under the direct supervision of the Health Services manager. The overall purpose of the Heart Health Plus Program is to reduce health inequity in areas with disproportionate burdens of heart disease and related risk factors. The incumbent will provide case management/care coordination services as delineated in the Heart Health Plus Program guidelines. The goal of the Senior Health Educator is to increase outreach and education to priority populations on chronic disease prevention and ways to address associated risk factors. The Senior Health Educator will also increase screenings and referrals to hypertension management, diabetes prevention, diabetes management, asthma, and tobacco cessation programs for priority populations. 

 

The employee will work promoting, maintaining, and improving individual and/or community health through SWAT (Students Working Against Tobacco) activities. The employee will collect and analyze data to identify individual and/or community needs prior to planning and implementing the program. This position is responsible for various aspects of the SWAT work plan, tracking and evaluation of projects, policy and advocacy work, meeting deliverables, and completing reports. Incumbents may also serve as a resource to assist individuals, other professionals and/or the community. This position requires the ability to work independently and to work with both youth and adults. Public speaking, coalition building, and program development skills experience is preferred. This position requires travel and some weekend and/or evening hours. This is an inhaled nicotine and tobacco-free position.

 

 

Students Working Against Tobacco (SWAT)

 

  • Serves as the SWAT Coordinator and leads this policy area for the tobacco prevention program

 

  • Maintains and builds relationships with public, private, and charter schools, as well as community-based youth organizations to establish and maintain SWAT groups

 

  • Establishes and maintains SWAT groups through activities such as, but not limited to, recruiting youth to participate in clubs, acting as a resource for SWAT Advisors,regularly attending SWAT meetings, and ensuring consistent communication with SWAT groups including progress assessments

 

  •  Works with local schools and school-based SWAT Advisors to plan and implement antitobacco activities

 

  •  Chaperones SWAT events and meetings per approved required activities for fiscal year

 

  •  Transports youth to statewide and regional events

 

  •  Serves as a mentor for the SWAT Chapter and assist by demonstrating and teaching responsible youth empowerment by increasing skills, knowledge of tobacco issues and prevention, and advocacy training

 

  •  Responsible for establishing and facilitating SWAT Leadership Group, meeting regularly to guide and coordinate efforts

 

Conducts screenings and blood pressure checks, as well as developing novel approaches to boost screening and referral rates among selected target populations. Incumbent identifies impediments and proposes strategies to prevent or address them. Incumbent keeps open lines of contact with Program Managers to keep all stakeholders informed of current events. Facilitate navigation of the healthcare and social service systems. Works with Gadsden County School Board, community organizations, and local stakeholders to increase countywide tobacco prevention activities for youth.
